Calcium Digital Health Platform Overview

Calcium Core is part of the Calcium digital health platform:

  • Calcium Core – used by healthcare providers to manage patients and pathways and access health data, medical records and analytics. Calcium Core may also be used by organizations and employers to create, assign and manage wellness programs for their members.
  • Calcium Super App – used by patients directed by providers using Calcium Core, by individual consumers without direct provider guidance, and can be used by employees whose employers use Calcium Core for wellness programs.
  • Calcium AI Studio – used by Calcium Core users to create, edit and manage healthcare and wellness pathways.

Purpose: the purpose of Calcium Core is to provide healthcare providers with a truly 360-degree view of their patient’s healthcare activity, medication adherence, treatment plan compliance, and more. Calcium Core is the provider’s insight hub and control center.

Calcium Core contains the following main segments

  1. Main Dashboard
  2. Members Tracking Dashboard – this section allows providers to analyze data and medical records for specific patients
  3. Analytics Dashboard – this section provides analytics reports for either individual patients or all patients in the provider’s group.
  4. Chat Dashboard – this section allows providers to securely chat with individual patients, using the chat feature in the Calcium App for patients.
  5. Invite Dashboard – this section allows providers invite patients to become members on the Calcium App or begin using recommended or required pathways.
  6. Pathways Dashboard – this section allows providers to manage, edit and create healthcare pathways. This segment specifically takes providers to the AI Studio which allows providers to automatically use OpenAI and ChatGPT to create draft pathways.
  7. Manage Dashboard – this section allows providers to invite and manage fellow administrators and teammates to use their Calcium Core account.
  8. Billing Dashboard – this section allows providers to view and manage their account and transaction history for their Calcium Core account, as well as change or cancel their plans.
  9. Get Help Dashboard – this section allows to access our knowledge base and help chatbot.

Core Organizations

Calcium Core allows users to be part of multiple organizations, such as a physician or nurse who may be part of different organizations or clinics within a large healthcare system.

Within an organization, Calcium Core users can also create Sub-Groups to better organize their team and/or patients. Organizations can create sub-groups in the Manage section of Calcium Core.

Creating Organizations

To create an organization, add additional organization or change an organization name, please reach out to the Calcium Customer Success team to create, add or change organizations.

Please contact the Customer Success team if you need to delete an existing organization.

Changing Organizations

From almost anywhere in Calcium Core, users can change the organization they are working on.

From the top navigation bar, Calcium Core users can click the “Change Organization” link to move from one organization to another.

If you have more than one organization in your user account, you can use the drop-down field to select a new organization.

Core User Profiles

Also, from almost every screen, you can access and edit your Calcium Core user profile. To edit your Calcium Core user profile, just click on your name and icon on the upper-right corner of the screen and select “Edit My Profile.”

Main Dashboard

Calcium Core provides a dashboard that providers can use to manage and analyze their patient engagement and patient care data more effectively.

The main dashboard page provides quick charts summarizing the provider group’s patients using the Calcium App. The charts may be clicked to access the data behind the charts.

Tracking Charts in Dashboard

The primary charts in the Calcium Core Main Dashboard screen visually summarizes the following tracking data:

  • Number of members (patients) connected to that provider or Calcium Core user.
  • Status of the member invites sent to the provider’s connected members (patients)
  • Alert statuses of the user’s or provider’s connected members (patients).

Top Pathways List

The Main Dashboard also lists the main pathways being used by members (patients) connected with the Calcium Core user.

Members Health Data Dashboard

The individual member health data dashboard in Calcium Core is the main area in which Core users (providers) can analyze their individual members and patients.

The individual member’s health data dashboard collects and organizes the member’s health data from an array of sources:

  • Electronic Health Records (EHR) – Calcium Core can access a member’s EHR and other medical records from over 95% of health systems, hospitals, and medical groups in the U.S.
  • Pathway Data – Calcium Core collects and organizes data from the pathways assigned to individual members and patients.
  • Calcium App Data – Calcium Core collects and organizes the health vitals and other health data recorded manually or automatically (via linked health devices) by the Calcium App.
  • Health Device Data – Calcium Core can collect and integrate health data from various wellness, fitness, and medical devices, such as Fitbit, Apple Watch, digital scales, digital blood pressure monitors and other devices.
  • Health App Data – Calcium Core also collects data from hundreds of popular health, fitness, and wellness apps, such as Apple Health, My Fitness Pal, Calm and Google Fit.

All this data, as well as other tools and resources, are organized in several tabs in the Member Health Data dashboard:

Summary Tab

The Members Dashboard includes search and filter options, which the Calcium Core user can utilize to quickly find a specific member or patient:

  • Search – the Search box allows Calcium Core users to find specific members (and patients) by their name.
  • Group – the Group filter allows Calcium Core users to view the members (patients) for a specific group or sub-group in their organization.
  • Alert Status – the Alert Status filter allows Calcium Core users to view the members (patients) based on their current alert status.

The Members dashboard provides a quick view table listing all the current members (and patients) of the Calcium Core user’s organization. This quick view table includes the following columns:

  • Email – the table may be sorted by the Email column. Clicking on an email address accesses the health data and electronic health records (EHR) for that member.
  • Name – the table may be sorted by the Name column. Clicking on a name accesses the health data and electronic health records (EHR) for that member.
  • DOB (Date of Birth) – the table may be sorted by the DOB (Date of Birth) column. Clicking on the date of birth accesses the health data and electronic health records (EHR) for that member.
  • Gender – the table may be sorted by the Gender column. Clicking on the gender of a specific member or patient accesses the health data and electronic health records (EHR) for that member.
  • Group – the Group column lists the group or sub-group to which a member or patient belongs.
  • Alert Status – the sortable Alert column indicates if the member or patient has an alert.
  • Last Lab Order Date – the sortable Last Lab Order Date column indicates when a lab was last ordered by the Calcium Core user.
  • Lab Report – the sortable Lab Report column indicates when a lab report was last received.
  • Care Plan – the Care Plan column indicates whether a member or patient has an assigned care plan. Clicking on the care plan icon, if any, for a specific member or patient accesses the health data and electronic health records (EHR) for that member.
  • Action – the Action column allows the Calcium Core user to act on that member or patient, such as by accessing their health data or medical records.

Member Health Data

As mentioned above, clicking on the individual member’s or patient’s name, email, date of birth or gender will access that member’s medical and health data. The Member Health Data dashboard includes the following information:

  • Summary Tab – the Summary Tab provides another quick overview of the specific member or patient. The Summary Tab includes…
    • Alerts – a quick tally chart of current alerts for that member
    • Last activity – the most recent time that member was active on the Calcium App
    • Health Index – the member’s current Health Index score and when the index score was last updated. The Calcium Health Index is a proprietary scoring system developed by Calcium LLC to measure the health activity and progress of a member, patient or consumer using the Calcium App.
  • Pinned Pathway Responses – responses from pathways assigned to a member or patient pinned for priority view.
  • Health Data Report – this section of the Member Health Data dashboard contains a series of chart tracking and monitoring the specific member’s vitals, conditions and progress. The charts in the Health Data Report section can be filtered to focus on a specific period of time, from the last 3 days to the past 3 years, for example.

Lab Results

The Lab Results tab provides Calcium Core users with access to the lab and imaging results for specific members and patients. These lab and imaging results are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Labs and imaging results produced by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Lab and imaging records, include Basic Metabolic Panel (BMP), Comprehensive Metabolic Panel (CMP), Lipid profile, Thyroid tests, Complete Blood Count (CBC) with or without White Blood Cell (WBC) Differential, Prothrombin Time (PT) with INR & Activated Partial Thromboplastin Time (PTT), Urinalysis (UA) and other tests.

Medications

The Medications tab provides Calcium Core users with access to the medication records for specific members and patients, particularly prescription medication.

These lab and imaging results are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Medication prescribed or recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Procedures

The Procedures tab provides Calcium Core users with access to the medical procedure records for specific members and patients.

These medical procedures records are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Medical and healthcare procedures conducted or recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Medical procedures include surgeries and treatments, such as appendectomy, biopsies, hernia repair, broken bone repair, knee replacement, organ transplants and other medical operations.

Encounters

The Encounters tab provides Calcium Core users with access to the healthcare and medical encounter records for specific members and patients.

These medical and healthcare encounter records are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Medical or healthcare encounters conducted or recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Healthcare and medical encounters include physical, medical appointments, lab visits and other recorded meetings with healthcare providers.

Diagnoses

The Diagnoses tab provides Calcium Core users with access to the healthcare and medical diagnoses records for specific members and patients.

These medical and healthcare diagnoses records are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Medical or healthcare encounters conducted or recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Allergies

The Allergies tab provides Calcium Core users with access to the allergy records for specific members and patients.

These allergy records are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Allergies encountered or recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Immunizations

The Immunizations tab provides Calcium Core users with access to the immunization records for specific members and patients.

These immunization records are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Immunizations provided or recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Vital Signs

The Vital Signs tab provides Calcium Core users with access to the vital signs records for specific members and patients.

These medical and healthcare vital signs records are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Vital signs recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Family & Social History

The Family History and Social History tabs provides Calcium Core users with access to the family medical history and health-related social factors for specific members and patients.

These records are obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Family medical history and health-related social factors recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Insurance

The Insurance tab provides Calcium Core users with access to healthcare and medical insurance information for specific members and patients.

A member’s health and medical insurance information is obtained from electronic health records (EHR) and medical records from health systems, medical groups and hospitals throughout the country. Insurance information collected or recorded by the Calcium Core user for a connected member (or patient) may also be uploaded into the Calcium Core database for that member – and shared with other authorized healthcare providers.

Pathways

The Pathways tab provides a record of a connected member’s (or patient’s) responses to prompts or questions from the assigned pathway. This dashboard contains the following elements:

  • Pathway Selector – this drop-down field allows Calcium Core users to choose the specific pathway to review.
  • Pathway Responses – for the specific pathway selected, this tab displays the member’s responses to the pathway assigned by the Calcium Core user to that member.
  • Trends – for the specific pathway selected, this tab displays and analyzes the response trends.

Shared Health Data

The Shared Health Data tab allows Calcium Core users to access the health data recorded from the connected member’s Calcium App. This data includes manually added by the member or patient to their Calcium App, as well as data automatically collected by the Calcium App from medical devices, fitness gadgets and other health apps.

Analytics

Analytics

The Analytics section allows Calcium Core users to review and analyze the health data collected for a connected member (patient) through the Calcium App.

The Analytics section allows users to review the data for individual members or for all the member connected with the Calcium Core user’s organization or group. This latter option allows Calcium Core users to review population health trends for their community of members and patients.

Chat

The Chat section allows providers to securely chat with individual patients, using the chat feature in the Calcium App for patients.

The Chat function on Calcium’s platform is secure and complies with all HIPAA and regulatory requirements.

Invite

The Invite section allows Calcium Core users to (1) invite individuals to become connected members on the Calcium App and/or (2) invite members to begin using recommended or required pathways.

This section allows Calcium Core users to review all the members that have been invited by the Core user’s organization. The list of invited members can also be sorted by the status of member invitation (i.e. Accepted, Pending, Declined or All).

Note that each line represents one member-pathway combination. So individual members can appear multiple times in this section, particularly if they have been assigned more than one pathway.

Inviting Members to Connect

To invite new members, click on the blue “Invite Members” button on the right side of the screen. Then complete the “Add Members” questions.

Note that Calcium Core users will need to provide detailed identity verification and contact information for the invited member, as Calcium Core users and their connected members (patients) will be able to access that member’s electronic health records and health data through the Calcium digital health platform.

Other Member Actions

The “Actions” column in the Member Invite table allows Calcium Core users to initiate communication- and pathway-related activities.

In particular, the clicking the three dots in the Actions column for an individual and assigned pathways allows the Calcium Core user to do the following:

  • Invite to Another Pathway – this option allows the Calcium Core user to assign another pathway to the individual member or patient. This will add a new member-pathway line to this table.
  • Remove from Pathway – this option allows the Calcium Core user to remove a particular member from the specific pathway assignment on that line.
  • Remove from Group – this option allows the Calcium Core user to remove a particular member from the current group indicated on that line.
  • Edit – this option allows the Calcium Core user to edit the member’s invitation information.
  • Email – this option allows the Calcium Core user to send an email to that connected member or patient.
  • Remove from Organization – this option removes the currently connected member from the current organization.

Pathways

The Pathways section allows Calcium Core users to access our pathway library and manage, create or edit pathways.

The primary view lists the Calcium Core user’s current available, published or draft pathways.

  • Filtering the list of pathways – To more efficiently manage the user’s library of pathways, this list can be filtered according to the assigned category for the pathway.
  • Sorting the list of pathways – The columns in the table of pathways are all sortable. Consequently, Calcium Core users can sort the list of pathways by name, category, length (number of steps), status (draft or published), or last published date.

Editing Pathways in AI Studio

To edit a pathway, Calcium Core users just need to click on the editing icon (pencil icon). This will take them to the Calcium AI Studio workspace for that selected pathway. Calcium Core users can also choose to edit a pathway manually by clicking the manual button.

Creating Pathways in AI Studio

Pathway creation and editing is done in the Calcium AI Studio (or manually), which is part of the Calcium digital health platform.

There are three basic ways that Calcium Core users can utilize to create new pathways for their members and patients:

  1. Cloning – Within the Calcium AI Studio, users can clone an existing pathway and re-configure it for their new requirements or use.
  2. Create Pathways with AI – At the heart of the Calcium AI Studio is our AI-powered pathway generator, which creates a detailed pathway for the Calcium Core user. This AI-generated pathway is delivered in draft mode and, after editing, updating and configuration by the Calcium Core user, can be published. Once a pathway is published, it can be assigned to connected members in the “Invite” section of Calcium Core
  3. Create Pathways Manually – Finally, Calcium Core users can also use the Calcium AI Studio to create new pathways manually. With this method, the Calcium AI studio will generate a proposed outline based on the Calcium Core user’s indicated parameters. However, it will be up to the user to manually structure and configure the pathways using the tools in the Calcium AI Studio.

To create a new pathway using any of the above methods, Calcium Core users should first click the “Create a Pathway” button.

Assigning a Pathway to a Patient

To assign a pathway to a patient, the Calcium Core user must go to the “Invite” section and invite a member to a specific pathway:

  • New Member – If the Calcium Core user is inviting a new member, the pathway may be assigned at the same time as the member is being invited to join.
  • Existing Member – If the Calcium Core user is assigning a pathway to an existing member, just click on the three (3) dots in the Actions column and select “Invite to Another Pathway.”

Note only published pathways may be assigned to members or patients. To publish a pathway that is currently in draft mode, select the editing icon for that draft pathway and then click the Publish icon (box with a diagonal up arrow).

Manage

The Manage section allows Calcium Core users to manage the structure and admin users in their organization’s account on the Calcium digital health platform. Please contact your Customer Success representative to update your organization’s setting.

Creating Sub-Groups

Within an organization, Calcium Core users can also create Sub-Groups to better organize their team and/or patients.

For example, a multi-location healthcare provider may create separate sub-groups for each location. Similarly, a healthcare provider may create different groups for different medical specialties.

Follow these steps to create sub-groups:

  1. Select the “SubGroups” tab in the Manage section of Calcium Core.
  2. Click the three dots next to the group name
  3. In the drop-down, select “Add Child Group”

Note: you can also create sub-sub-groups within your sub-group.

Deleting Sub-Groups

To delete a sub-group, click on the three dots next to the sub-group and select “Delete Group.”

Adding Team Members as Admins

Organizational colleagues can be added either individually or in bulk. To add team members as fellow admins, first go to the Manage section of Calcium Core and select the “Group Admins” tab.

Then click on the “Invite Admins” button, which will display the Invite Admins form.

General

The following is information on general and miscellaneous elements and features of the Calcium Core platform.

Exporting Data

Many of the data and records in Calcium Core may be exported by authorized Calcium Core users, for their connected members and patients.

Calcium Core data, which includes medical records and pathway logs, can be exported to a CSV file. To export Calcium Core data to a CSV file, you first must go to the specific data set, such as the Lab Reports tab or Pathways tab. From there, simply click the “Export to CSV” link to initiate exporting the data to a PDF file.

Logging Out

It’s best practice to log out of your Calcium Core account when you’re not using it. To sign out or log out of your Calcium Core account, click your profile icon on the upper-right corner of your Calcium Core screen; then select “Sign Out.”
